Population Health Specialist, Independence Blue Cross

Siah McCabe is an alumna of the Trinity Health Administrative Fellowship Program. Post fellowship, Siah transitioned into her role as Project Lead for Practice Transformation with Mercy Accountable Care, providing project and program management support to the 10 Mercy Physician Network Practices. Following this role, she served as the Director for Strategic Partnerships at a Non-Profit organization and is currently working as a population health specialist with Independence Blue Cross.

Prior to living and working in the Greater Philadelphia area, Siah grew up in Columbus, Ohio before moving to Cincinnati, Ohio where she attended the University of Cincinnati, obtaining an undergraduate degree in Psychology. Siah then continued her education attending Xavier University where she received MHSA and MBA degrees. Siah's efforts span beyond her professional career in her passion for servant leadership. She worked with Trinity Health's Office of Diversity and Inclusion to assist in forming and supporting D&I work as well as local procurement at the Regional and Local levels. Siah regularly volunteered with the Health Promoters Program providing preventative health, dental and vision screenings to West Philadelphia and South Philadelphia community members. Siah was a member of Together for West Philadelphia, a non-profit supporting West Philadelphia, regular volunteer at Mastery Charter School, a former member of the Junior League of Philadelphia and spent Sunday’s prior to the pandemic serving dinner at “Our Brother’s Place” one of the Bethesda Projects shelters.

Siah currently holds a leadership role with the Chamber of Commerce for Greater Philadelphia’s Young Professional’s Council as an Advisory board member and Events Committee Co-Chair and now sits on the Young Professionals Advisory Board for the Bethesda Project.
